Thirteen golden verbs

The repo ships 50 MCP tools. tools/list shows 13 by default. The others can still be called by name. PUAX_TOOL_SURFACE=full lists all of them.

When a tick needs a follow-up tool (failure switch, breakthrough, or saving reasoning before compaction), that tool is added to this session's list and the server sends notifications/tools/list_changed. HTTP sessions do not share that list.

puax_switch_on_failure and get_role_with_methodology are still there. Without a hook they are the manual gearbox. With a hook you do not memorize them first.
